At Portner Orthopedic Rehabilitation, we respect our patients’ time by maintaining a precise schedule, and we count on punctual arrival to keep our day flowing smoothly. YOUR APPOINTMENT DAY
Relax! One of our goals is to make every visit comfortable and pleasant by treating you with respect and kindness. Be content with the fact that you are doing something positive for yourself and let our team worry about finding you effective, nonsurgical orthopedic solutions. You are about to be professionally guided by a highly experienced team of specialists on your road to recovery!
ARRIVAL INFORMATION
For your first visit to the clinic, please arrive 15-20 minutes before your appointment time in order to complete the registration process. For follow-up visits, please arrive promptly at the appointed time. Typically, for initial visits you can expect to spend approximately one hour at the office and 30-45 minutes for subsequent appointments. The actual amount of time may vary, sometimes due to factors beyond our control.
COME PREPARED
Please bring a picture I.D., Social Security number, and insurance information, as well as pertinent details about your condition including X-rays, MRI and EMG results, current medications, and any past medical records.
We encourage you to take notes about your orthopedic condition, including exactly where you hurt, the date your issue began, what you where doing at the time, any test results or treatments attempted, and what activities and positions seem to make your condition worse or better. Any details on previous similar problems will also help us understand and diagnose your condition.
Because our waiting room area is small, we ask that you limit the number of people you bring with you. If you have children, please try to arrange for a babysitter; of course, we will understand if you must bring them along. If you have trouble with English, please bring an interpreter. If you need assistance in any way, your helper is welcome. If you are a minor, we will need your parent or guardian to sign your documents.
DRESSING FOR THE OCCASION
Please wear comfortable, loose-fitting clothing. Shorts or baggy pants are practical for a knee exam, for example. Swimsuits (two-piece for women) beneath your clothes can be convenient. Wearing slippers or sandals that are easy to remove is very practical. If you are a runner, athlete, or have a foot problem, bring along your athletic shoes.

If possible, please print out the new patient paperwork, fill it out at home, and bring it with you to your first visit.
